The NHS Health/Hospital Passport is an editable document designed for people with learning disabilities and those who support them.
It allows individuals, family members, and carers to record important information including:
• Health and social care needs
• Likes, dislikes, and personal preferences
• Reasonable adjustments required during care
The passport helps ensure that anyone with a learning disability is well supported, understood, and treated with dignity when accessing health services.
How to use it:
Once completed, please send your Hospital Passport to huh-tr.learningdisability@nhs.net and we will upload it securely to your records, so it is available to staff when you need care.
